On July 1, 2004 The Community Hospital and Mercy Health System Western Ohio merged to form one regional
healthcare system affiliated with Catholic Healthcare Partners (CHP). As a result of the consolidation, most of the corporations previously organized under Community Hospital Health Services Foundation (CHF) were realigned under Community Mercy Health Partners (CMHP), the newly formed corporation.
CHF retained one former subsidiary, CH Venture Co., Inc., a for-profit corporation that owned interest in an ambulatory surgery center (Springfield Surgery Center). In addition, Community Hospital’s ownership interests in Clark County Dialysis Facility, Community Physicians Dialysis Center (Springfield) and Community Physicians Dialysis Center Fairborn were transferred to CHF. Finally, in 2008, CHF formed a joint venture with physicians to open Community Physicians Dialysis Center Urbana. The chart depicts these relationships.

Currently, all three Community Physicians Dialysis facilities provide in-center kidney dialysis and operate under the business name of Midwest Dialysis Centers with CHF as the managing partner. Clark County Dialysis Facility, a non
profit corporation wholly owned by CHF, operates a contract with CMHP to provide inpatient, acute dialysis at CMHP’s three hospital locations.
The former Community Hospital Independent Endowmwnd Fund (CHIEF) merged into CHF on January 1, 2009.
